Job Description:
A company is looking for a Registered Nurse Triage & Transitions
Ambulatory to manage patient calls and coordinate care
transitions.
Key Responsibilities
Perform clinical triage and comprehensive assessments using
established protocols
Manage transitions of care to ensure appropriate follow-up and
reduce readmission risks
Educate patients and families on health maintenance and management
of conditions
Required Qualifications
Graduate of an accredited school of nursing
Current/valid Registered Nurse license from the state licensing
board
Basic Life Support (BLS) or Advanced Care Life Support (ACLS)
certification required
One year of related experience in a clinic or acute hospital
nursing setting
Bachelor of Science in Nursing is preferred
