We are seeking a compassionate RN to join our hospice team at Heart of Hospice, LLC.As a Hospice RN, you will have the opportunity to develop meaningful relationships with patients and their families, providing end-of-life care with empathy and understanding.Key responsibilities include:

• Conducting initial nursing evaluations to determine eligibility for hospice services
• Identifying patients' physical, psychosocial, and environmental needs
• Initiating and coordinating individualized care plans
• Collaborating with interdisciplinary teams to provide comprehensive patient careRequirements:
  • Current RN licensure in the state of practice
  • One year of clinical experience
